Ensuring a successful transition into adulthood

At this stage, your child has become an adult, and as such, they will be taking the lead in managing their healthcare. During this time, they will be learning what the world holds for them, preparing themselves for the rest of their lives, and making decisions, both good and bad, for their own life. We are here to help them during these years with navigating the healthcare system. It’s during this time that we begin working with your adult child to start the process of transitioning from the pediatric healthcare model to an adult provider who will be able to care for their specific needs as they continue to grow and develop. At CPAM, we continue to care for our patients through age 21 or completion of a college degree or training program, whichever is later.
